Vitalis Health & Home Care was born from a simple and powerful vision: to create an integrated and compassionate provider of quality clinical and care services that makes a difference in people’s lives. We have been serving customers with a wide range of health and home care needs since 2015 and have supported the journeys and successful health outcomes of thousands of people. We have grown quickly to become the standard for quality in-home clinical and care services, including: - Hospital in the Home (HITH) - Chemo in the Home - Palliative Care in the Home - Home Care - Telephonic services (discharge support, chronic disease management, health coaching and care coordination) Our mission is to deliver the highest standard of care with compassion and respect to support people’s quality of life. Vitalis supports insurers, hospitals, specialists, GPs, corporations, retirement villages and individuals with programs and services that promote self-management and preventative care, reducing avoidable hospitalisations. We also support successful transitions from hospital to home, reducing readmission rates and improving well-being, through the provision of telephonic nurse support and clinical services such as HITH, Chemo at Home and REHAB+. Our home care division enables and empowers people to continue living independently in their own home for as long as possible as they age or need to manage chronic health conditions.
