Mountain Family Health Centers

Our mission is to provide high quality, integrated primary medical, behavioral, and dental health care in the communities we serve, with special consideration for the medically underserved and uninsured, regardless of ability to pay. MFHC is a Federally Qualified Health Center (FQHC) with locations in Glenwood Springs, Rifle, Edwards, Basalt, Parachute, and Avon in Colorado. Our service area includes Garfield, Eagle, Pitkin, and Rio Blanco counties.
