Advanced Home Health Care Agency is a licensed Medicare and Medicaid certified home health care agency. We are committed to high quality skilled nursing care and rehabilitation and support services to our patient. Our goal is to foster our patient's optimal health, function and well-being so they can remain in their homes leading independent lives. The patient's primary care nurse works closely with the patient's physician to develop a plan of care that is specific to the patient's unique needs. We provide the following services: Skilled Nursing, Behavioral Health, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, Home Health Aides and Adult Foster Care. We have nurses that speak the following languages and understand their cultural needs: Russian, Ukrainian, Polish, Spanish and Greek.
