Our comprehensive behavior change platform combines ADA, Motivation Surveys, Patient Reported Outcomes Surveys and a Clinic Based Intervention with remote monitoring. Our platform integrates seamlessly with clinic workflow with text to enroll on boarding in a modular implementation, in clinic, at home, or at hospital discharge. Platform is HIPAA compliant, FHIR enabled, and integrates with any EHR. Providers harness the preventScripts Bundle in order to grow additional prevention related revenues to 10,000$ per provider per month for a 100 patient panel.
