Heartwood Care Group

Heartwood Care Group is a successful social care provider who work closely with local authorities to deliver day to day care and support to young people in and around the London and Essex areas. We provide a ‘wrap around service’ which includes Residential Care for SEBD young people aged between eleven to seventeen, 24/7 Supported Living Services for young people aged between sixteen to eighteen and Outreach services with accommodation for young people from seventeen to twenty four. Our person centred approach allows us to work collaboratively with local authorise and other social care professionals to provide an environment where the young people we support have the best chance to achieve positive outcomes where our aim is for our young people to move on into independence successfully.
