HN is an award-winning UK-based healthcare company that delivers AI guided case-finding, clinical coaching, remote monitoring, and virtual ward solutions to the NHS. Established by clinicians and researchers at Sweden’s Karolinska Medical Institute, our journey in the UK began in 2015. Working in collaboration with the Nuffield Trust, we launched a large trial into the effectiveness of clinical coaching, which has shown exceptional outcomes for patients and the NHS. Since our UK launch, we have developed into an award-winning NHS partner and since March 2020, have been supported by the NHS Innovation Accelerator to scale nationally. The HN approach is unique. We blend our advanced data scientific and AI capabilities with digital services, which are delivered by highly experienced healthcare professionals. This provides the NHS and patients with integrated and comprehensive solutions. We provide practical applications of population health management, going beyond just identifying high-cost, high-need patients and actually intervening to support them to improve their health outcomes and reduce their care consumption. Our vision is to empower patients, by engaging them in their own health and care, thereby reducing their need of NHS services and creating sustainable healthcare for all.
