One Homecare Solutions is an integrated homecare service provider focused on meeting the needs of Health Plans by serving their members through a single source solution. We manage our patients’ complex and chronic needs from infusion care to nursing and durable medical equipment services where it matters most – at home. Care provided in the home helps promote quality of life, improved health for our patients and supports the family unit, ultimately resulting in low hospital re-admission rates and reduced costs for Health plans. One Homecare Solutions is 20 years in the making. Through our experience we discovered the need for an integrated solution to address the demands for care required by health plan members being served by the traditional, fragmented, and inefficient homecare service model. We created One Homecare Solutions to provide one, cost effective solution to serve the growing demand for quality homecare. We have applied our knowledge and improved methods that allow us to fully coordinate care, enhance performance scores and improve overall service.
