Our Mission Horizon Hospice and Palliative Care, Chicago's first hospice, is a not-for-profit, community-based organization whose mission is to provide comfort for the dying, to preserve dignity at the end of life for all in need of services, and to educate the community. Our Vision A nationally recognized leader that provides excellent and innovative hospice and palliative care service throughout the Chicago area. Our History Horizon was founded by Ada Addington and three other volunteers in 1978 and has been providing end-of-life care services to people throughout the metropolitan area for more than 30 years. While the organization is licensed to provide care throughout Cook, Lake, DuPage and Will counties, it primarily serves an urban population in Chicago. Horizon recently acquired a south suburban branch to provide services in the Flossmoor, Homewood, and Olympia Fields area. The south suburban office was known for many years as "Braveheart," a volunteer hospice that served the south suburbs. The branch also has a Service Board of dedicated community leaders who provide fundraising assistance and community connections. Horizon is licensed to serve patients and their loved ones in Cook, Will, DuPage and Lake counties.
