Founded in 1977, Health Center Partners of Southern California is a private, nonprofit association comprised of 16 community clinics and health centers providing primary care for San Diego, Imperial and Riverside counties. A premier consortium of primary health care organizations, Health Center Partners is an advocate for its members providing them with the voice and power to serve vulnerable populations.

HCP is a champion for community health care and blends its years of experience, richly talented staff and deep knowledge of its members to delivery programs resources and health policy designed to enrich the patient experience.
