Established in 1978, Elderwood provides skilled nursing care, specialized subacute care, rehabilitation, assisted living, independent living and memory care services in Western and Central New York as well as in Rhode Island and Pennsylvania. The company is owned by Post Acute Partners, which also operates the managed long term care plan Elderwood Health Plan, Elderwood Transportation and Woodmark Pharmacy, and has grown to become the largest network of rehabilitation, skilled nursing, assisted living and independent living communities in Western New York. Elderwood is well known to, and highly regarded by, the hospitals and physician groups in the communities that each location serves. Customer satisfaction is rigorously tracked through robust surveys and interviews. Based on our latest data, 96 percent of residents said they were pleased with the care they received at an Elderwood community. Elderwood’s continuum of care also includes advanced specialized services in cardiac rehab, oncology, orthopedic rehab, respiratory care, SMARTS® Express Care, wound care, adult medical day services, cardiac telemetry services, outpatient therapy, respite care and Seasons® Memory Care.
