Community Care Plan (CCP), 'the health plan with a heart,' was introduced in 2000 as South Florida Community Care Network (SFCCN), the first safety-net hospital-owned Provider Service Network. CCP serves 100,000+ members enrolled in Medicaid, Florida Healthy Kids, sponsored programs, and self-insured employee health plans. CCP is owned by Memorial Healthcare System and Broward Health.
