Care Resources is a long-term health care provider that enables elderly adults to live at home for as long as possible. Care Resources offers managed, comprehensive health care through the Program of All-inclusive Care for the Elderly (PACE). Care Resources provides all primary & specialty care, a recreational day center, home care, medical transportation, prescription drugs and many other services for eligible participants. Eligibility for the Program: - 55 years old or older - Lives in Service Area (Kent County) - Meets Nursing Facility Level of Care - Able to Live Safely in Home at Time of Admission No out of pocket cost for program if someone is Dual Eligible(Medicare/Medicaid) or just Medicaid.
