Care Alliance Health Center

Care Alliance Health Center is a non-profit, federally qualified community health center providing comprehensive primary and preventive medical, dental, behavioral health, patient advocacy, and wrap-around services to families and individuals, regardless of the ability to pay. Last year, Care Alliance served more than 16,000 Clevelanders at two health center facilities, as well as through a mobile health center and outreach programming. Recognized as a Patient Centered Medical Home, Care Alliance provides our patient population, including members of the community experiencing homelessness and poverty, with critical services to address their unique healthcare needs. Care Alliance prioritizes building trusting relationships with our Cleveland family to provide inclusive, meaningful, supportive services for all.
