Heartland Health Centers Chicago

Heartland Health Centers (HHC) provide high-quality, affordable healthcare through a network of community and school-based health centers on the north side of Chicago. HHC also provides primary medical care to patients at two community mental health centers on the north side. Our mission is to ensure that our patients, many of whom are uninsured and live in low-income communities, have a medical home where they can access medical, mental health and dental care in one location.
