Community Health Works

Community Health Works serves the Central Georgia region as a community-based health organization with a vision of better health for all people through communities working together. Our Mission is regional integration of whole person healthcare. By testing, incubating and implementing programs and initiatives aimed at eliminating health disparities and improving both health outcomes and quality of health care in central Georgia, Community Health Works serves as a Regional Center for Health Innovation. Organized in 1999, CHW has grown into a multi-county collaborative that seeks to improve the health of local residents and the capacity of the local health care community. The original core service area of CHW includes Bibb, Crawford, Houston, Jones, Monroe, Peach and Twiggs Counties. Over the past decade we have expanded our reach to more than 1/3 of the state, including, but not limited to public health district 5-1 and 5-2. Our four main areas of focus include: improving access to care, improving health literacy among youth and adults, improving the quality and coordination of health care providers and health care systems, and improving coordination and regional integration of health information technology.
