WHO WE ARECenterLight Health System is a leading non-profit provider of health and related services to older adults in need across the New York metropolitan area. Teamcare, CenterLight Healthcare’s flagship Program of All-Inclusive Care for the Elderly (PACE), is one of the oldest and largest such programs in the United States. WHO WE SERVECenterLight Teamcare is a Medicare-Medicaid plan for participants aged 55 years and older, who require long-term care.
