At Maple City Health Care Center, our mission is to foster healthy community in our neighborhoods by providing and promoting affordable, accessible, and integrated quality care, and to articulate and promote our experience as a sustainable model.
Our medical sites are family practices that offer integrated medical care including obstetrics, midwifery, pediatrics, geriatrics, mental health counseling, social work, chronic disease case management, addictions recovery, nutritional services, healthy living groups, group care programs, and tattoo removal. The dental site offers primary dentistry for all patients, ages 3 and up.
